ABSTRACT

Objective:To investigate the effect of lncRNA AC132217.4 on the proliferation and invasion of liver cancer MHCC97-H cells and its molecular mechanism. Methods:The TCGA database was used to analyze the differential expression of AC132217.4 in liver cancer tissue and adjacent tissue, and to analyze the relationship between the expression level of AC132217.4 and the overall survival of liver cancer patients. Transfection of pcDNA-AC132217.4 plasmid into MHCC97-H cells was defined as AC132217.4 group, transfection of pcDNA plasmid into MHCC97-H cells was defined as negative control (NC) group, respectively. The proliferation and invasion ability of MHCC97-H cells were detected by MTT method and Matrigel invasion assay. The binding site between AC132217.4 and miR-18a-5p was analyzed by starBase v2.0 software and dual luciferase reporter gene assay. Real-time quantitative PCR (RT-qPCR) detected the differential expression of miR-18a-5p in the two groups of MHCC97-H cells. The expression of epithelial-mesenchymal transition protein was detected by Western-blotting. Measurement data with normal distribution were expressed as mean±standard deviation ( ± s), and t-test was used for comparison between the two groups. Results:Compared with adjacent tissues, the expression of AC132217.4 was down-regulated in liver cancer tissues ( P<0.01). Compared with liver cancer patients with low expression of AC132217.4, the overall survival of liver cancer patients with high AC132217.4 expression was longer ( P<0.05). The pcDNA-AC132217.4 plasmid significantly inhibited the proliferation of MHCC97-H cells ( P<0.05). The number of invasive cells in the NC group and AC132217.4 group were (131.30±12.55) and (37.45±7.77), respectively. The pcDNA-AC132217.4 plasmid significantly inhibited the invasive ability of MHCC97-H cells ( t=6.36, P<0.01). AC132217.4 directly complemented miR-18a-5p ( P<0.01). The expression of miR-18a-5p in MHCC97-H cells in AC132217.4 group (1.04±0.30) was significantly lower than that in NC group (6.13±0.75) ( t=6.27, P<0.01). Compared with the NC group, the expressions of epithelial phenotype proteins Cytokeratin and Claudin-1 in MHCC97-H cells in AC132217.4 group were up-regulated, while the expressions of mesenchymal phenotype proteins Vimentin, Slug and Snail were down-regulated. Conclusions:The expression of AC132217.4 is low in liver cancer tissue, and it is related to the overall survival of liver cancer patients. AC132217.4 might inhibit the proliferation and invasion of liver cancer MHCC97-H cells by sponge miR-18a-5p.

ABSTRACT

OBJECTIVE To estab lish the pre paration method of clopidogrel active thiol metabolite (CATM),and to provide reference for the synthesis of cis-CATM. METHODS CATM was prepared ,separated and purified with isolated rat liver perfusion and ChromCore 120 C18 preparative column ,using(S)-2-oxo-clopidogrel as substrate. The target compounds were identified by mass spectrometry and nuclear magnetic resonance spectroscopy. The retention time of the active configuration of CATM in the human body (cis-CATM)were compared to confirm the proportion of active configuration in the target product. RESULTS The conversion rate of the target product was 11.71%. The target products were identified as CATM by MS and 1H-NMR. Peak 2-peak 5 of CATM were four stereoisomers. The retention time of them were 21.3,22.3,26.5,27.3 min. The peak area ratios of them were 7.13%,7.23%,63.52%,14.97%,respectively. Based on that retention time of the active configuration of CATM in human body was 26.3 min,the active cis-stereoisomer in the target product CATM accounted for 63.52%. CONCLUSIONS This method is low-cost ,simple, and can prepare CATM with higher active configuration.

ABSTRACT

Objective:To investigate frailty status of the elderly patients with leukoaraiosis (LA) and to analyze the correlation between white matter hyperintensities and their frailty status. Methods:From June 2019 to September 2020, a total of 162 patients with leukoaraiosis over 65 years old were screened by cranial MRI.The Fried frailty phenotype was used to evaluate their frailty status. The Fazekas scale scoring method was used to independently assess the periventricular white matter hyperintense (PVH) and deep white matter hyperintense (DWMH) by the cranial MRI images.SPSS 22.0 software was used for statistical analysis.ANOVA test was used to compare normal distribution data between groups, and Kruskal-Wallis H test was used to compare non-normal distribution data between groups.Spearman rank correlation analysis was used to analyze the correlation between PVH and DWMH scores and Fried frailty phenotype score. Results:Among 162 elderly patients with leukoaraiosis, 46 patients (28.40%) were non-frailty, 76 patients (46.91%)were pre-frailty and 40 patients (24.69%) were frailty. There were statistically significant differences in age( F=9.382, P<0.01), number of chronic diseases( H=10.736, P<0.01), number of medication ( H=15.927, P<0.01) and mini-nutritional assessment short form (MNA-SF) scores( F=5.263, P<0.01) among older LA patients with different frailty phenotype. There was statistical difference in PVH scores in elderly LA patients with different frailty phenotype (χ 2=108.537, P<0.01), but no significant difference in DWMH scores (χ 2=4.239, P>0.05). Spearman correlation analysis showed significant positive correlation between PVH score and frailty phenotype score in elderly LA patients ( r=0.718, P<0.001). Conclusion:Elderly LA patients have a high incidence of frailty, which may be related to aging, multi-disease coexistence, multiple medication, nutritional risk and other factors. The occurrence of weakness in elderly LA patients is related to periventricular white matter lesions, and the more serious the white matter damage, the more obvious the degree of frailty.

ABSTRACT

Objective:To investigate the clinical characteristics and influencing factors of pain symptoms in patients with idiopathic Parkinson′s disease. Methods:The King′s Parkinson′s Disease Pain Scale (KPPS) was used to evaluate pain of 106 patients with Parkinson′s disease. The Pittsburgh Sleep Quality Index (PSQI), the Fatigue Scale-14 (FS-14), the Unified Parkinson′s Disease Rating Scale Ⅲ, Hoehn-Yahr scale (H-Y), Mini-Mental State Examination (MMSE), Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were used to assess the sleep status of patients, the degree of fatigue, motor function, severity of symptoms, cognitive function, anxiety and depression. Fifty-eight patients were followed up for three to six months. Results:The incidence of pain in Parkinson′s disease patients was 50.0% (53/106), of which skeletal muscle pain was the most common. Parkinson′s disease patients with a longer course of disease were more likely to have pain [course of disease in Parkinson′s disease with pain was 3.0 (1.5, 5.0) years, in Parkinson′s disease without pain was 2.0 (1.5, 2.5) years, Z=-2.0, P=0.046]. Male patients had more severe pain than female patients [KPPS scores in males were 14.5 (8.0, 21.5), in females were 10.0(4.0, 15.3), Z=-2.81, P=0.005]. In the first evaluation, the H-Y grading and the FS-14 scores of Parkinson′s disease patients with pain were significantly higher than those without pain [H-Y grading in Parkinson′s disease patients with pain was 2.0 (1.5, 2.5), in Parkinson′s disease patients without pain was 2.0 (1.5, 2.0), Z=-2.02, P=0.043; FS-14 scores in patients with pain were 10.0 (8.0, 11.0), in patients without pain was 8.0 (5.0, 10.0), Z=-3.32, P=0.001]. The KPPS scores were positively correlated with the scores of HAMA and HAMD ( r=0.39, P=0.005; r=0.38, P=0.007). Binary Logistic regression analysis showed that Parkinson′s disease patients with higher scores of FS-14 had an increased risk of developing pain ( OR=1.27, 95% CI: 1.09-1.48, P=0.002). The changes of KPPS scores were associated with the changes of PSQI and FS-14 scores ( r=0.54, P=0.002; r=0.50, P=0.003). The decrease of KPPS scores was only positively correlated with the decrease of FS-14 scores when the drug and medication status remained unchanged ( r=0.421, P=0.045). Multiple linear regression analysis showed that the decrease of FS-14 scores was associated with the decrease of the KPPS scores ( OR=2.02, P=0.033). Conclusions:Parkinson′s disease patients have a high incidence of pain, and fatigue is a factor for predicting the occurrence and outcome of pain in Parkinson′s disease. The severity and change of pain in Parkinson′s disease patients are related to anxiety, depression, sleep and fatigue, suggesting that there may be a common pathogenesis of pain, emotion, sleep and fatigue in Parkinson′s disease patients.

ABSTRACT

Objective:to review the clinical features, diagnosis and treatment of spinal Rosai-Dorfman disease(RDD).Methods:we conducted a systemic review and collected the cases reported from 2010. The key words were Rosai-Dorfman disease, spine/central nervous system. We screened both English and Chinese database. There were 43 reports finally included in the study, containing 52 cases. We distracted the information of interest and, subsequently, analyzed the harvested data using specific statistical software packages. The study focused on the summary and description of the clinical features, diagnosis and treatment and prognosis of spinal RDD.Results:The included articles reported 52 cases. The average age was 32.1±17.1 years (ranging from 6 to 76 years old). The ratio of male to female was 1.9/1. The median follow-up period was 19.9 months. The initial symptoms of 41 patients (78.8%) were spinal lesion-related. The cases with painless lymph nodes enlargement, other organ lesions and abnormal lab tests were 11.5%, 36.5% and 23.1%, respectively. The frequent infringed segments were cervical (43.1%) and thoracic (39.2%) spine. 53.2% lesions were dura-based, while 17.0% and 10.6% for bone and cord, respectively. Surgery was the mainstream of the treatment armamentarium, composing 83.7% cases, among which 46.3% underwent total resection. Cases only treated with radiotherapy, chemotherapy and steroids were 10.2%. Very Few cases remitted spontaneously (2.0%). The risk of recurrence and occurrence at other vertebral levels was 22.0%.Conclusion:It is rare for spinal involvement of RDD. This entity has no pathognomonic clinical and imaging features. RDD has a tendency of multi-organ involvement and recurrence. Surgery remains the mainstay of the treatment, but the efficacy of other adjuvant therapies is not sure. A wait and watch strategy is employed for asymptomatic patients.

ABSTRACT

Background and purpose:Radiomics refers to the comprehensive quantification of human tissues through assessing a large number of quantitative image features. Radiomics approach is used to decode tumor phenotypes and predict treatment outcomes. Here we present a study investigating radiomic analysis to assess normal liver features and predict chemotherapy-associated liver injury. Methods:Gastric cancer patients treated with surgery and adjuvant chemotherapy were enrolled in this study retrospectively. CT images were obtained before chemotherapy. The whole liver organ was delineated by radiation oncologists. Images were extracted and filtered by radiomic approach to extract radiomic features. Clustering was performed to reveal clusters of patients with similar radiomic expression patterns. Chi-squared tests were used to assess the association of radiomic data with clinical data and chemotherapy-related liver injury. Results:Radiomic features of 73 patients were clustered into two clusters. A significant association with gender (P=0.004, chi-squared test) was observed, where in male showed a higher presence in cluster Ⅰ. Incidence of abnormal liver function after chemotherapy was 48.7% in cluster I and 67.6% in cluster Ⅱ, respectively (Δ=18.9%). Clinical data including age, gender, chemotherapy modality, number of chemotherapy cycles, HBV infection history, HBs-antigen presence were not associated with liver function abnormalities after chemotherapy. Accuracy of radiomic analysis to predict liver injury is 0.59.Conclusion:Radiomic approach revealed different imaging features of liver between men and women. It could help to predict chemotherapy-associated liver injury. It is feasible to use radiomics approach to decode normal liver features and predict treatment-associated liver injury.

ABSTRACT

Objective To investigate the effects of dopaminergic medication on decision-making un-der ambiguity in patients with early Parkinson's disease( PD) . Methods Using Iowa Gambling Task ( IGT) for 24 early non-medication idiopathic PD patients( Hoehn and Yahr Scale≤Ⅱlevel) ,24 early idiopathic PD patients with regular dopaminergic medication and also for 24 healthy controls( HC) whose age, gender, and education match to PD patients to test their ability of decision-making under ambiguity. Results The results showed non-medication PD group showed impairments on digtal span and verbal fluency and decision-making task. There was significant difference in IGT task scores among the three groups(F=6.024, P=0.004) . The total net scores of advantageous choices in IGT were significantly lower in non-medication PD group( (-4.50 ±22.19) scores) than medication PD group((8.83±23.24)scores) and healthy group((15.92±15.77) scores) . The difference of net scores in block1 to block5 between non-medication PD group and medication PD group was gradually increased, and the difference of net scores in block5 was significant(P0.05) . As the game processing, medication PD group gradually shifted their se-lections toward the advantageous choices. But non-medication group did not exhibit this shift pattern and the performance was much poorer. Meanwhile, the study also indicated the total net scores of advantageous choices for non-medication PD group was positive correlation to the MoCA scores ( r=0.614, P=0.001). Conclusion The present study has shown that non-medication PD group has impairment in decision-making under ambiguity risk condition and prefer to choose risky options. when exogenous complement dopaminergic medication, the risk decision-making ability of medication PD group has been improved.

ABSTRACT

STUDY DESIGN: A retrospective clinical review. PURPOSE: To describe the incidence of cerebrospinal fluid leakage (CSFL) after thoracic decompression and examine the CSFL predisposing clinical factors. OVERVIEW OF LITERATURE: CSFL is a common complication following thoracic decompression but has not been sufficiently addressed in former studies. METHODS: A cohort of 362 cases of thoracic decompression from February of 2005 to June of 2013 was examined. The case medical records were reviewed and the occurrence of CSFL and the related clinical parameters were noted. The incidence of CSFL for the entire cohort and each surgical approach were described. Besides, the relationship between CSFL and other clinical parameters were assessed, of which odds ratio values of all CSFL-associated parameters were calculated using multivariate logistic regression analysis. RESULTS: The incidence of CSFL for the entire cohort was 32.3%. Different surgical approaches had different incidences of CSFL, and circumferential decompression had the highest incidence. Though many different clinical parameters were related to the occurrences of CSFL, being older than 52 years, having ossification of the posterior longitudinal ligament or having longer operative segments than 3 vertebrae were significant risk factors for CSFL (p <0.05). Besides, surgeries on the mid-thoracic spine had an increased risk of CSFL (p <0.05). CONCLUSIONS: From our analysis, CSFL was a common complication after thoracic decompression with the incidence of up to 32.3%. This study identified the predisposing clinical factors, and spinal surgeons should be aware of these risk factors to reduce its incidence.

ABSTRACT

STUDY DESIGN: A prospective imaging study. PURPOSE: To characterize the distribution of the global sagittal postural patterns in asymptomatic Chinese adults using Roussouly classification. OVERVIEW OF LITERATURE: The norms of sagittal parameters in asymptomatic Chinese population have been previously described, but no report described their global sagittal postural patterns as characterized by Roussouly classification. METHODS: A cohort of 272 asymptomatic Chinese adults was recruited. Data was assimilated by reviewing the films for each subject. Sagittal parameters were measured and sagittal postural patterns were then determined according to Roussouly classification. The pattern distributions were compared across genders within the study cohort. We also compared the data across different ethnicities from our study and a previous study to further characterize Chinese sagittal postures. RESULTS: The cohort included 161 males and 111 females, with mean age of 23.2±4.4 years. The average descriptive results were as below: pelvic incidence (PI) 46.4°±9.6°, thoracic kyphosis (TK) 24.2°±9.0°, lumbar lordosis (LL) 50.6°±10.6°, sacral slope (SS) 37.2°±7.6°, pelvic tilt (PT) 9.4°±6.8°, spinosacral angle (SSA) 131.1°±7.5° and sagittal vertical axis (SVA) 17.24±32.36 mm. Despite a significant difference between two genders in LL, PI, SSA, and SVA, no difference was found in the distribution of Roussouly types among them. 47.8% of our cohort belonged to Roussouly type 3, while type 1, 2 and 4 comprised 23.2%, 14.0% and 15.1% of the subjects, respectively. Roussouly classification was capable of categorizing sagittal parameters except for the PT. This study also found that 4.4% of the recruited subjects belonged to the C7-anterior subgroup. CONCLUSIONS: From a characterization of the sagittal postural patterns of asymptomatic Chinese adults using Roussouly classification, the distribution was similar between Chinese males and females; however, from a cross-study comparison, it was different between asymptomatic Chinese and Caucasian adults, with a higher proportion of Roussouly type 3 in Chinese adults.

ABSTRACT

Objective To investigate the episodic memory monitoring in patients with white matter lesions (WML) and explore the relevance of episodic memory disorder and memory monitoring and the possible mechanism. Methods The feeling of knowing-episodic memory (FOK-EM) paradigm was adopted and subsequently administered in 30 WML patients and 30 control participants who were matched in age and educational level. Results Compared with control group (cued recall 4.37 ± 1.59;recognition 8.73 ± 1.05),the episodic memories of WML patients were significantly impaired on the cued recall performance (3.30 ± 1.51,t =2.666,P < 0.05) and recognition performance (7.23 ± 0.77,t =6.306,P < 0.01).Furthermore,the accuracy of FOK judgment (-0.13 ± 0.31),the correct judgment and correct recognition of FOK-EM (23.16% ±5.96%) and the correct judgment and false recognition of FOK-EM (34.26% ± 7.65%) in the WML patients group were significantly different compared with the control group (the accuracy of FOK judgment:0.34 ± 0.27,t =6.280,P < 0.01;the correct judgment and correct recognition:35.12% ± 6.82%,t =7.234,P < 0.01;the correct judgment and false recognition:19.27% ±6.50%,t =-8.174,P < 0.01).In addition, the correct judgment and false recognition performance were positively correlated with the severity of periventricular lesions and deep white matter lesions in the WMLpatients group (r=0.716,P<0.01;r=0.598,P<0.01).Conclusions The WML patients demonstrated an overestimation of their recognition ability of episodic memory. Moreover, this impairment of memory monitoring was correlated with the deficit of executive function caused by the damage of the prefrontal-subcortical circuit, indicating that this mechanism could be an influential factor of episodic memory disorder in WML.

ABSTRACT

Objective To explore scrambled biological motion perception in Parkinson’s disease (PD). Meth?ods Thirty individuals with idiopathic PD and matched health controls (HCs) underwent discrimination task. The severi?ty of disease was assessed based on the Hohen and Yahr scale. Results Compared with“PSE=0”,there was a significant negative PSE (the point of subjective equality) (-0.15 ± 0.18) for HCs ( t=4.56, P<0.001), suggesting a temporal dilation effect of the upright scrambled biological motion sequences in HCs. There was a significant difference in the PSE be?tween PD patients(t=3.72,P<0.001) and HCs. Patients in the PD group were then divided into two subgroups accord?ing to their status of disease. There were significant differences in PSE between early-stage PD patients (stage 1-2) (0.02±0.26) and HCs (P<0.01), and between mid-stage PD patients (stage 2.5-3) (0.06±0.12) and HCs (P<0.01). There was no significant difference in PSE between early-stage and mid-stage PD patients (P=0.61). Conclusion The present study suggests that there is a scrambled biological motion perception impairment in PD at the very early stage, which might be attributed to the dysfunction of the prefrontal lobe.

ABSTRACT

Objective:To explore the relationship between spino-pelvic sagittal and coronal parameters in Lenke 1 group of adolescent idiopathic scoliosis (AIS).Methods:The subjects were retrospectively col-lected from 2005 to 2013.On the posteroanterior and lateral radiographs, apical vertebra ( AV) , Cobb an-gle of main thoracic curve (MT), pelvic incidence (PI), C7 translation ratio (C7TR) and other sagittal parameters were measured and recorded.Comparison and correlation studies were conducted between these parameters using specific softwares. Results: In the study, 51 subjects, including 18 males and 33 fe-males, were recruited, aged (14.9 ±2.0) years averagely. The apical vertebra ranged from T7 to T11,with mean MT being 49.6闭±16.7闭, and mean PI 44.7°±6.7°.Significant correlation existed between PI and PT, SS, LL, as well as between LL and SS, TK ( P <0.05) .Significant differences were found in TK, LL and SS among the different LM groups, but no difference in the other sagittal parameters.AV had no significant correlation with any sagittal parameter.MT was significantly correlated with TK, LL and SS, but its correlation with PI was not significant. Conclusion:Most of sagittal parameters were significantly corre-lated in Lenke 1 adolescent idiopathic scoliosis, forming a regulation chain of spine-pelvic sagittal balance on the basis of PI.Significant correlation exists between some sagittal and coronal parameters.
